According to what I've read, some people consider the DOF markings on
most lenses to be a bit optimistic, and prefer to take a
more conservative approach. In your example, with the lens set at f16 they
would use the f11 marks on the lens. This is what I do, particularly if I
think I might be wanting to make a large print later.

The hyperfocal distance is the distance that actually falls at the
focusing index mark when you set the DOF scale to give you a range to
infinity on the far side.
