Stefan: The focusing mount you need is the 16464(OTZFO) for the Tele-elmar
lenshead. The price here in the US  is $100+-30, depending on condition and
finish; the black one is usually more expensive. However,  it is hard to
find  this item on the used market. Try a store in Berlin or Frankfurt(?) if
it is not available in Scandinavia.
The 16471 mentioned by Marc is an extension tube inserted between the  OTZFO
and the adapter and is used for close up work.
